Job Advert:
Active Monitoring Practitioner (AMP)

Grade:  NJC Point 24 £21,745 pro rata
Hours:                  24 hours per week – possible additional hours
Fixed Term Funding until 31 March 2018
Location:            Within GP surgeries across Cynon Valley, Taff Ely and Rhondda areas

 
We are a mental health and wellbeing charity and we deliver a range of community based services across Merthyr Tydfil and Rhondda Cynon Taff.

Active Monitoring provides support to people who visit their GP with symptoms of common emotional needs including stress, anxiety and depression.

 This service will offer one-to-one support for up to 8 weeks.   An AMP will work with Clients to help them understand their symptoms and apply self-directed support.

 We are looking for Counsellors to work within the Active Monitoring service by applying psycho-educational self-directed interventions to people experiencing mild-to-moderate mental health and wellbeing needs.
