What are your priorities for older people’s services in Cwm Taf?

We need your views on our plans and priorities for older people’s health and social care services in Cwm Taf
Because of changes in the law, and the growing number of older people in our area, your local councils and health board are working together to look at new plans and priorities for older people’s health and social care services across Cwm Taf.

We want older people to have longer, healthier and happier lives, and to stay as independent as they can be for as long as possible.

We think this means we need to change the types of services we provide, and the way we provide them. We need your views on our plans and priorities for the future of older people’s services, to see whether you think they’ll achieve what we want for older people in our area.

We’ll be asking the public for their views from 27th July - 18th September 2015.

Rydyn ni eisiau'ch sylwadau am ein cynlluniau a'n blaenoriaethau ar gyfer gwasanaethau iechyd a gofal cymdeithasol i bobl hŷn yng Nghwm Taf

Oherwydd newidiadau yn y gyfraith a'r nifer gynyddol o bobl hŷn yn ein hardal ni, mae eich bwrdd iechyd a'ch cynghorau lleol yn cydweithio i edrych ar gynlluniau a blaenoriaethau newydd ar gyfer gwasanaethau iechyd a gofal cymdeithasol i bobl hŷn ar draws Cwm Taf.

Rydyn ni am i bobl hŷn fyw bywydau hirach, iachach a hapusach, ac aros mor annibynnol ag y gallan nhw fod am gyhyd â phosibl.

Rydyn ni o'r farn bod hyn yn golygu bydd angen i ni newid y mathau o wasanaethau rydyn ni'n eu darparu, a sut rydyn ni'n eu darparu nhw. Rydyn ni angen eich barn am ein cynlluniau a'n blaenoriaethau ar gyfer dyfodol y gwasanaethau i bobl hŷn, i weld a ydych chi o'r farn y byddan nhw'n cyflawni'r hyn rydyn ni'i eisiau ar gyfer y bobl hŷn yn ein hardal.

Byddwn ni'n gofyn i'r cyhoedd am eu sylwadau o 27 Gorffennaf tan 18 Medi 2015.
